Protecting your personal health information is of utmost importance to us. Medical Mutual

The Fund uses the Medical Mutual PPO provider network, allowing you to see any provider you choose without needing a referral to see a Specialist or designating a Primary Care Physician before seeking care.

Medical Mutual has special arrangements and receives certain discounts from participating providers, which means if you use an in-network provider, you also pay a lower coinsurance percentage. Participants residing outside of Ohio have only the in-network level of benefits. The out-of-network level of benefits does not apply to out-of-state participants. CVS Caremark®

The Ohio Conference of Teamsters & Industry Health and Welfare Fund has partnered with CVS Caremark to manage your prescription benefits.

Register at Caremark.com, where you can see plan information, check drug cost and coverage, find personalized drug savings opportunities and more. For more information and answers to commonly asked questions, visit Caremark.com/HelpCenter or call CVS Caremark customer care at 1-844-449-0353. To have your prescription filled, simply present your CVS Caremark member ID card at any in-network pharmacy nationwide (including those inside Target stores).

The Teamsters Ohio Contractors Association Health & Welfare Fund uses Optum Rx as its Plan’s Prescription Benefit Manager to provide you the Prescription Drug benefits.

Should you have any questions about your pharmacy or participating pharmacies in your area, please feel free to contact Optum RX customer service department using the phone number listed on the back of your Optum RX ID card. To have your prescription filled, simply present your drug identification card, and make your co-payment.

Delta Dental of Ohio

The plan uses the Delta Dental PPO ™ and Delta Dental Premier ® networks. Seeing a dentist in the Delta Dental PPO network will offer you the best savings. You may go to any licensed dentist; however you may pay more out of pocket if you choose a nonparticipating dentist.

  • Find a Delta Dental provider online or by calling customer service at 1-800-524-0149.

  • Make an appointment to see your Delta Dental dentist, and let them know you are a Delta Dental of Ohio member.

  • If your dentist is not currently in a Delta Dental network, you can refer your dentist on our website by entering your dentist’s information in the dentist referral section.

National Vision Administrators

National Vision Administrators (NVA) has been selected as a partner to provide you and your family vision benefits. You can go to any vision care provider, however, using a NVA network provider will save you money and give you the greatest benefit under the Plan.

Finding a Network Provider is easy. Visit www.e-nva.com and select the Find a Provider link. Enter your group number and your search parameters. You don’t even need an ID Card when you see a provider. Just tell your vision care provider that you have benefits through NVA.
